I wish to enroll / re-enroll myself / my child for music tuition at Stratford Music. I understand that all missed lessons need to be paid for unless at least 48hrs notice is given. In the event that my teacher is unavailable to attend lessons, the School will make all efforts to supply a substitute teacher, or
organize a make-up lesson. In the event that I /my child does not turn up for lessons for a period of two weeks without notice, I understand that our
lesson time may be given to someone on the waiting list. I am also aware that lessons may be videotaped or viewed on
camera as part of the “Child Safety Practices” in place at the School. A school term will normally have 9 to 11 weeks worth of
lessons. I understand lessons will need to be paid in advance, or via the Schools direct debit option in order to secure a
tutor and my lesson time. By signing below you're indicating that you have read the terms and conditions of
